Updated 2/28/2020  I authorize Moxie Management Group, LLC (“Moxie Management”) to debit the bank account or credit card indicated in this web form, for the noted amount on today's date and subsequent billing dates based on the term selected. This payment is for the Supercuts Unlimited service noted on supercutsunlimited.com. Supercuts Unlimited service is only valid for person named on account. Supercuts Unlimited services is not valid for Synthetic and Human Hair Wigs or beard trim. I understand that refunds are not permitted. I understand that Supercuts Unlimited payments are not valid for loyalty purchases. However, exceptions may be made on a case-by-case basis. For monthly memberships I agree to a minimum 3-month billing schedule term starting with today's date.  I understand that to cancel my membership, I must contact Moxie Management with written notice via email (scunlimited@moxiemg.com) 7 business days prior to my next bill date. A service fee of up-to one term payment may be charged for memberships canceled prior to the minimum commitment. For 3-6 month memberships a minimum 2 term commitment is required. The additional requirements noted above for of monthly memberships apply. For 1 year memberships a minimum 1 term commitment is required. The additional requirements noted above of monthly memberships apply. For Unlimited Color services, single service per visit. Only valid with a call ahead appointment. Valid on Glazing, Root Touch-ups, Camo Color and Foil up-to 22. Additional services or add-ons extra. See participating locations for color during signup. Moxie Management has the right to change or terminate the Supercuts Unlimited program at any time. If terminated, paid memberships will be canceled immediately and you will lose access to Supercuts Unlimited. Moxie Management has the right to adjust membership price at any time, effective the next billing cycle. If a participating location closes, members will be transferred to the nearest open participating location. Terminations or changes will be communicated via email and at supercutsunlimited.com. Moxie Management may suspend billing cycles if a majority of participating locations closes due to National or State mandate. Billing cycles will resume once mandates are lifted. I understand that because this is an electronic transaction, these funds may be withdrawn from my account as soon as the above noted transaction date. I agree that payments for billing schedules with common bill dates may be consolidated into a single payment. services and the website are only provided on an “as is” and “as available basis”, services are only provided during regular store hours, and appointment days and times are not guaranteed. I affirm that all information I have provided upon registration is complete, accurate, and true and that I will not share my account information, login, or password with others. In the case of a credit card or ACH Transaction being rejected for Non-Sufficient Funds (NSF) I understand that Moxie Management may at its discretion attempt to process the charge again for the next 30 days. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this credit card or bank account, and that I will not dispute the payment with my Credit Card Company or Bank, so long as the transaction corresponds to the terms indicated in this web form. If your credit or debit card expires, you authorize us to obtain a new expiration date from the card issuer, if we choose to do so, and/or continue billing the card in accordance with the terms of this agreement (whether or not we have obtained a new expiration date). A third-party “PaySimple” manages subscription charges paysimple.com. Supercutsunlimited.com contains links to third-party websites. Moxie Management has no control over these links and is not bound to the third party’s terms or privacy policy. Moxie Management is not responsible for transactions conducted on third-party websites.
